Summary

The Linkage to Care Specialist (LTC-S) Project aims to improve timely linkage for patients newly diagnosed with HIV through client-centered, resiliency-based counseling, and support beginning immediately after a positive HIV test result.

Intervention Outcomes

94% of participants attended their first HIV medical visit within 3 months of diagnosis.
92% of participants were successfully retained for a year following linkage to care.
Among individuals retained in care, 94% achieved viral suppression.
